New Patient Information Package
Be Prepared
New patients to Atlanta Brain and Spine Care are asked to completed New Patient Information Packet and return to our office at least one week prior to your appointment. Copies of your medical records should be sent to our office from your primary or referring physician. In addition, we also ask you bring along x-rays, MRI, or CT Scans, insurance cards, and picture identification to your first visit.
Failure to not have required information may result in a delay in your visit or a rescheduled appointment.
