Recovery after any type of surgery requires rest, avoidance of strenuous activities, and taking things easy so the body can heal properly. A common pitfall can be trying to do too much too soon. It’s important to follow all of the surgeon’s post-surgery instructions completely for optimal healing. Spine surgery has some specific restrictions that should be followed.
What is the BLT restriction?
The BLT restriction means no bending, lifting, or twisting post-surgery. These movements place the most stress on the spine and can compromise the surgical repairs that were made and interfere with the healing process. Bending forward, or spinal flexion, strains muscles and ligaments that were cut or pushed aside during the procedure. Bending may cause herniation of a repaired disc or stress the hardware, such as rods and screws, that was used in a spinal fusion. These reinjuries make recovery more difficult.
Lifting restrictions following spinal surgery are typically limited to no more than 5-10 pounds for the first few weeks. The surgeon will gradually increase the weight limit as healing progresses. Heavy chores and activities, such as vacuuming or yard work, are usually restricted until approved by the doctor. When patients are allowed to lift, they should take care to use good posture and proper lifting techniques. This includes keeping the back straight, engaging the core muscles, and using the legs.
Twisting the spine post-surgery puts stress on the surgical site and supporting structures. Patients should avoid chores and activities that require rotating the trunk. The “log roll” technique works well when getting out of bed following surgery, preventing unnecessary twisting.
What are some additional post-surgical restrictions?

In the early days following spine surgery, there may be restrictions on soaking in the bath tub, hot tub, or swimming. Showers are generally allowed. Submerging the new incision increases the risk of infection.
Driving may be restricted for 3-4 weeks after spine surgery. Patients should not drive until they have finished taking strong pain medications that could cause drowsiness. Mobility will be limited and reflexes and reaction times will be slower than normal. Sudden braking or jolts may cause reinjury to healing tissues. Patients should wait for their doctor’s approval before driving again.
Patients should wait for their doctor to approve their return to work. This will greatly depend on the type of spine surgery they had and their job responsibilities. People who have more physical jobs will generally not be cleared to return to work as soon as those who have desk jobs. Some people will be able to return to light duties with some restrictions.
